No Patient-Provider Relationship

Use of this website, including submission of any forms, does not establish a patient-provider, therapist-client, or physician-patient relationship. A treatment relationship is only established through our formal admissions process, which includes clinical assessment, consent to treatment, and execution of applicable treatment agreements.
